Hole in glass using Patty Gray dam

Hi,
I emailed Patty but have not received a response. So, I am trying here. I use Patty's 8x10" dam a lot. All the first pieces I've made came out well. The last two pieces have big holes and bubbles in them. I use papyrus as she taught me in her class and store my molds in their original boxes. Please see pics and advise what may have gone wrong. Thank you!

Not enough glass. The total glass is less than 6mm thick and that causes the holes.

One solution is to not fire as hot. Also, it helps to do a bubble hold at around 1230F for at least half an hour.

There was 6mm, 3mm clear base and 3mm strips covering the top.

Still not enough glass for the temperature you fired to.
